Health care professionals in Wisconsin are working together to make health care safer.

But they cannot do it alone.

Physicians, nurses, and pharmacists are just a few of the professionals on a patient’s health care team. Each has a very specific role to play in ensuring that the right medicine or treatment is given to the patient. The patient and their family also have an important role to play in ensuring their own safety.

In July 2005, a statewide organization, "Safe Care Wisconsin," was formed with the purpose of improving patient safety by giving patients and their families tools they can use to ensure their own safety.

Safe Care Wisconsin's first project emphasizes the importance of writing down and carrying an accurate list of medications. Other projects are also under development.

Safe Care Wisconsin members represent hospitals, physicians, insurers, quality improvement organizations, patient safety organizations, and others committed to ensuring the safety of patients throughout Wisconsin.
